摘要：Objectives:To examine the patient outcomes of a comprehensive health education plus village health volunteer monitoring support program on older adults with knee osteoarthritis who are overweight and have type 2 ***:Quasi-experimental,two-group,pretest-posttest design was *** random geographic cluster sampling,older adults who met the inclusion criteria were assigned to two groups,62 in the intervention group and 71 in the control *** intervention group received the comprehensive health education plus village health volunteer monitoring support *** control group received only the comprehensive health education *** outcomes,including blood glucose level,knee pain and range of motion,body weight,physical ability(Timed Up and Go Test),fatigue,depressive symptoms,quality of sleep,and quality of life,were assessed at baseline,3 months,and 6 months after enrollment and analyzed using descriptive statistics and multivariate analysis of ***:One hundred and ten participants completed the study(55 participants in each group).Most patient outcomes in the two groups showed general improvement with statistical significance(P<0.001):pain decreased,physical ability improved,less depressive symptoms and fatigue,quality of life and sleep *** interaction effect(the group and time)demonstrated statistically significant differences between the intervention and control groups on pain,knee range of motion,and fatigue according to each time follow-up(P<0.001).Conclusions:The comprehensive health education plus village health volunteer monitoring support program promotes good patient outcomes in this *** integrated health education resource and support for older adults with knee osteoarthritis and type 2 diabetes is an effective,non-surgical treatment that highlights professional nursing roles and non-professional rolesdvillage health *** should consider implementing a health education plus monito

摘要：A discrete insulin infusion based on long-time interval measurement is the classic technique for diabetes treatment. Nevertheless, in this research, a closed-loop control system was proposed for continuous drug infusion to overcome the drawbacks of these typical discrete methods and develop more practical diabetes therapy systems. A blood glucose-insulin system was implemented relying on continuous insulin injection model. Based on this model, two controllers were designed to deal with the control dilemma of the resulting highly nonlinear plant. The controllers designed in this paper are: proportional integral derivative (PID), and sliding table controllers. Simulation results have shown that the sliding table controller can outperform the PID controller even with severe circumstances of disturbance in glucose, such as exercise, delay or noise in glucose sensor and nutrition mixed meal absorption at meal times.

摘要：Background: The lower extremity muscle strength has been shown to be weaker in patients with diabetes mellitus, primarily due to the effects of diabetic polyneuropathy, a common diabetic complication. With respect to assessments of the lower extremity muscle strength, reference values for the knee extension force in healthy Japanese subjects have been reported. The clinical significance of applying knee extension force in the assessment of efficacy of exercise therapy and patient education has also been demonstrated, and the use of an individualized approach in physical exercise based on the quantitative assessment of the lower extremity muscle strength is becoming one of the most important strategies for promoting effective rehabilitation. Methods: The study is a multicenter, cross-sectional investigation. We will measure the knee extension force and evaluate the diabetic polyneuropathy in patients with type 2 diabetes using uniform equipment and methodology at all centers. We will compare the knee extension force between the subjects with and without diabetic polyneuropathy and will investigate the associations between the knee extension force and both glycemic control and exercise habits. The trial registration is UMIN CTR (UMIN000002810). Discussion: The lower extremity muscle strength has been insufficiently investigated in Japanese patients with diabetes. This study was designed to establish reference values for the knee extension force in Japanese diabetic patients with and without diabetic polyneuropathy.

摘要：A plant fermentation was carried out by Yeast and Lactobacilli against single cellular algae (f-Chlorella). These materials were proved by as safe in animal safety experiment. We sought to look into changes of immune-competent cells that commonly utilized f-Chlorella including after administration of immno-suppressed animals. The effects by f-Chlorella on the regurational effect to the host were measured. Our results showed that f-Chlorella regulated the level of lymphocytes, while conventional-Chlorella worked to adjust the level of granulocytes. In our clinical study with 20 healthy volunteers, granulocyte and lymphocyte ratio was obtained as neutral in the peripheral blood. In rodents, immune-compromised host as well as normal animal were prepared with cancer chemotherapeutic agent (Mytomycin-C), and then tried to rescue the immune-competent rebel. Our observations showed that both Chlorella derivatives regulated in number and functions. The effect was more prominent in f-Chlorella than that of conventional one. We discussed the significance and mechanism of f-Chlorella on the level of leukocyte subsets in number and function. These effects were considered to bring the potential elevation of antibody secreting cell by the localized heamolysis in gel method. We also proposed an idea that exhibited tonic effects were brought via activating the complement components in the serum. Moreover, we tried to access further to the anti-oxidative activities of this f-Chlorella. This modification brought to the significant lifted up for immune competent cells and anti-oxidative activity for phagocytic cells.

摘要：Recently, the anti-glycation and anticancer properties of curcumin longa and okra seed extract were studied alone and also in combination with the well-established drug metformin. The combined effect of curcumin with metformin and metformin with okra seed extract was found to be highly efficacious in inhibiting Advanced Glycation End-products （AGEs）. In order to understand the mechanistic implications of curcumin combined with metformin and its enhanced anti-glycation activity, a Curcumin-Metformin Adduct was chemically synthesized. This adduct was fully characterized by thin-layer chromatography, Nano Drop spectrophotometry and electrospray-ionization mass spectrometry. The adduct may be helpful not only in elucidating the mechanism of anti-glycation and anti-cancer activities but also in studying the role of curcumin in binding of AI3-oligomers and disaggregating fibrillar formation in Alzheimer＇s disease.

摘要：A plant material consisted by Family Poaceae was fermented by Yeast and Lactobaccilli (U-164). This material was proved by as safe in animal safety experiment for oral administration. In order to prove the effect of U-164 against physiological function, the animal and human trials were set up to look into mainly leukocyte functions. In animal experiment, anti-oxidative effect and antibody response in immune-compromised host and diabetes meritus were made up. For human use, peripheral lymphocyte in number and subset ratio were followed up to one month after administration. In order to understand its effect, human complement component analysis was made by immune-electrophoresis. Our results showed that U-164 augmented the level of lymphocytes, while U-164 down regulated the level of granulocytes. In our clinical study with 19 healthy volunteers, granulocyte and lymphocyte ratio was obtained as neutral in peripheral blood being increased significantly 30 days after the ingestion of U-164. In experimental animal study, the compromised host as well as normal animal was administered with cancer chemotherapeutic agent (Mytomycin-C). Our observations showed against antibody producing cell, this material recovered the antibody production in the host compromising the immure responsiveness. We also proposed an idea that U-164 exhibited tonic effects via activating complement components. Moreover, we tried to access further to the anti-oxidative activities of this U-164. 摘要：Background: In the past few decades, the incidence of cardiometabolic diseases and disorders of the liver, kidney, pancreas, heart, and lung, have been increasing in Western and Asian countries, including Japan. Numerous factors, including abnormal body weight (obesity, overweight, or underweight), infrequent exercise, and other unfavorable lifestyle factors (e.g., smoking and heavy alcohol drinking) have been proposed as risk factors for the development and the progression of diseases, such as type 2 diabetes, hypertension, and dyslipidemia, which ultimately lead to impaired organ function and possibly death. However, the mechanisms that link these risk factors with diseases are still poorly understood, and the potential treatments, including pharmacotherapy and diet, have not been fully evaluated. Methods: In 2011, we established a new collaborative research program, the Saitama Cardiometabolic Disease and Organ Impairment Study (SCDOIS). This multidisciplinary observational epidemiological research study was designed to cover several high-profile diseases and some traditional fields of internal medicine, as well as apparently unrelated fields and particular lifestyle factors, such as unhealthy eating behaviors. In a series of studies, apparently healthy subjects who underwent a regular medical checkup were retrospectively identified based on the results of their medical checkups. In this way, the incidence, prevalence, causality, and clinical relevance of specific conditions and diseases have been investigated in cross-sectional analyses of 100,000 - 200,000 adults, and in longitudinal studies of several thousand subjects who underwent medical checkups multiple times. Discussion: This article describes the background, rationale, purpose, and methods of the SCDOIS.
