摘要：随着人们对冠状动脉粥样硬化性心脏病,特别是急性冠脉综合征和支架置入后血栓形成机制的深入理解,抗血小板药物在上述领域得到了越来越广泛的应用。然而,传统的抗血小板药物阿司匹林和噻氯匹定作用较为温和,且均具有潜在的严重不良反应:阿司匹林有胃肠道不适和出血,噻氯匹定有骨髓抑制,皮疹和腹泻。氯吡格雷是新的噻吩并吡啶衍生物,化学结构上与噻氯匹定类似,是抑制二磷酸腺苷诱导的血小板聚集的抗血小板药。近年来,有三个大规模随机临床试验证实了氯吡格雷不但作用强于上述两种药物,而且安全性更好。 CAPRIE是一项随机、双盲的国际性研究,设计的目的是比较氯吡格雷(75mg/d)与阿司匹林(325mg/d)降低缺血性中风、心肌梗死或血管性死亡危险的作用,同时评价二者的相对安全性。试验共入选19 185例患者,平均随访1.91年。结果发现氯吡格雷组的缺血性中风。
