摘要：Background： Despite the rapid growth in the incidence of acute myocardial infarction （AMI） in China, there is limited information about patients＇ experiences after AMI hospitalization, especially on long-tern1 adverse events and patient-reported outcomes （PROs）. Methods： The China Patient-centered Evaluative Assessment of Cardiac Events （PEACE）-Prospective AMI Study will enroll 4000 consecutive AM I patients from 53 diverse hospitals across China and follow them longitudinally for 12 months to docunlent their treatment, recovery, and outcomes. Details of patients＇ medical history, treatment, and in-hospital outcomes are abstracted from medical charts. Comprehensive baseline interviews are being conducted to characterize patient demographics, risk factors, presentation, and healthcare utilization. As part of these interviews, validated instruments are administered to measure PROs, including quality of life, symptoms, mood, cognition, and sexual activity. Follow-up interviews, measuring PROs, medication adherence, risk factor control, and collecting hospitalization events are conducted at 1, 6, and 12 months after discharge. Supporting documents for potential outcomes are collected for adjudication by clinicians at the National Coordinating Center. Blood and urine samples are also obtained at baseline, 1 - and 12-month follow-up. In addition, we are conducting a survey of participating hospitals to characterize their organizational characteristics. Conclusion： The China PEACE-Prospective AMI study will be uniquely positioned to generate new information regarding patient＇s experiences and outcomes alter AMI in China and seiwe as a foundation for quality improveinent activities.

摘要：背景：尽管人们对动脉粥样硬化血栓形成已有新的认识，但是，预测女性心血管事件的方法在很大程度上仍依赖于传统的危险因素。目的：根据新旧危险因素对女性心血管疾病危险评估法进行改进并予验证。设计、地点及参试者：于24558名健康的≥45岁的美国女性中对35个危险因素进行评估分析。随访中位时间为10．2年（截至2004年5月），观察心血管事件（复合终点包括心肌梗死、缺血性卒中、冠状动脉血运重建以及心血管死亡）的发生率。随机使用三分之二女性队列（推导队列，n=16400）的资料推导新的危险计算方法，然后将其结果与其余三分之一女性队列（验证队列，n=8158）的实际情况进行比较，借此对新计算方法的有效性进行验证。主要观测指标：根据BIC（Bayes Information Criterion）最小化原则在推导队列中建立最适简易预测模型。在验证队列中比较10年心血管事件的预测发生率与实际发生率。对新的计算方法和根据成人胆固醇治疗计划Ⅲ（ATP-Ⅲ）危险评分协变量产生的模型亦进行比较。结果：与ATP-Ⅲ危险评分协变量模型相比，在推导队列中，有1个最适模型（模型A）和1个临床简化模型（模型B，雷诺危险评分）具有较低的BIC评分。在验证队列中，应用模型A及模型B时，所有拟和、判别及校准指标均获改善。举例而言，根据ATP-Ⅲ危险评分标准，在非糖尿病女性参试者中，有603人10年心血管疾病危险为5％～10％，156人10年心血管疾病危险为10％～20％；而根据模型A，则可将其中的379人（50％）重新归类为更符合实际事件发生率的更高或更低的危险级别；根据年龄、收缩压、糖化血红蛋白A1c（如果患糖尿病）、吸烟、总胆固醇和高密度脂蛋白胆固醇、超敏C-反应蛋白以及60岁前心肌梗死家族史产生的简化模型B也能达到相似的效果。对于ATP-Ⅲ评分划归为极低危险组女性，采用两种新的计算方法并无明显不同。结论：我们推出了两种可大大改善心血管事件危险预测准确性的临床计算方法。验证结果表明，这两种方法可将40％～50％的传统中危组女性重新划归为高危组或低危组。

摘要：背景：心力衰竭（heart failure，HF）这一综合征的表现多种多样。虽然医生可以根据患者射血分数（ejection fraction，EF）和舒张功能加以诊断，但是，目前有关社区人群心力衰竭特点的资料却寥寥无几，而且多数相关研究都是以医院为单位、有赖于实验室检查的回顾性研究。此外，鲜有研究采用标准化技术对患者舒张功能进行系统评价。目的：对社区心力衰竭患者的EF、心室舒张功能和脑型利钠肽（brain natriuretic peptide，BNP）水平进行前瞻性研究。主要观测指标：超声心动图测得的EF值和舒张功能、血BNP水平、6个月死亡率。设计、地点及参试者：于2003年9月10日至2005年10月27日在奥姆斯特德郡（Olmsted County）招募HF新发病例及陈旧病例（住院患者或门诊患者）。应用超声心动图测定EF和舒张功能，检测BNP水平。结果：共有556例参试者在确诊HF时接受了超声心动图检查。其中EF值正常（〉150％）者有308例（55％），多见于年长、女性和既往无心肌梗死病史的患者（P均〈0．001）。242例患者（44％）存在单纯舒张功能不全（即舒张功能不全，但EF值正常）。与EF正常的患者相比，EF降低的患者合并中、重度舒张功能不全的比例较高（风险比，1．67；95％可信区间[confidence interval，CI]，1．11～2．51；P=0．01）。EF降低以及舒张功能不全均与BNP水平升高独立相关。EF正常者及降低者6个月死亡率均为16％（对年龄和性别进行校正后风险比为0．85；95％CI，0．61—1．19；P=0．33）。结论：社区中半数以上的心力衰竭患者EF值正常，40％以上的患者存在单纯舒张功能不全。射血分数和舒张功能不全均与BNP水平升高独立相关。与EF降低的心力衰竭患者相似，EF正常的心力衰竭患者死亡率也较高。

摘要：Background: Although studies have examined the incidence of stroke in heart failure(HF), their findings are inconsistent and difficult to interpret because of heterogeneity in study design and population. Although HF remains a highly fatal disease, the excess mortality imparted from stroke is unknown. Methods: A random sample of cases of HF from 1979 to 1999 was identified and validated according to Framingham criteria. Strokes were identified by screening medical diagnoses and subsequent physician validation. Stroke risk in HF was compared with the risk in the general population with standardized morbidity ratios. Associations between selected characteristics and stroke were examined using proportional hazards regression. Results: The study cohort included 630 persons with incident HF. During a median of 4.3 years of follow-up, 102(16%) experienced an ischemic stroke. Heart failure was associated with a 17.4-fold increased risk for stroke compared with the general population in the first 30 days after HF diagnosis and remained elevated during 5 years of follow-up. Older persons with prior stroke or diabetes were more likely to experience stroke after HF diagnosis. Persons with stroke after HF were 2.31 times more likely to die compared with persons without stroke. Conclusions: In the community, persons with HF have a large increase in the risk for ischemic stroke compared with the general population. Stroke results in a >2-fold increase in mortality. Thus, prevention of stroke has the potential to improve survival among patients with HF, particularly among the elderly and those with diabetes or prior stroke.
